Prior to your first appointment, a new patient information packet will be mailed to you upon scheduling a new patient appointment. Please plan to complete the forms and bring them with you to your first scheduled appointment.
About Your First Visit
Prior to your first appointment, a new patient information packet will be mailed to you. The packet will contain Insurance and Health Information Forms (for your convenience, you may also download these forms from our Website). Please complete these forms and bring them with you to your first appointment.
When you arrive for your first visit, be prepared to complete all Insurance and Health Information Forms if you haven't already done so. The completed forms provide us with information we need to begin your initial dental exam -- or treatment.
Your scheduled appointment time has been reserved specifically for you. Your initial visit will last two hours. During this time we will get to know you and complete our Comprehensive Exam that includes:
- Reviewing your completed health history forms and compile your medical and dental history
- Answering any questions and address any concerns you may have
- Discussing any pain or problems you may be experiencing
- Thoroughly examining your teeth and gum tissue
- Completing an oral cancer screening
- Taking X-rays of your teeth (as needed)
- Completing a TMJ (temporomandibular or jaw joint) exam
- Outlining any existing dental problems
- Preparing and discussing treatment options and preparing a treatment plan, if necessary
This complete and thorough examination will confirm the precise condition of your overall oral health. It's designed to help you:
- Alleviate major oral health problems in the future. Our comprehensive initial exam provides a baseline of where you are today, so any change or progress of an existing disease can be monitored, helping to avoid or minimize any major problems
- Plan and budget for your dental treatment, if additional treatment is necessary
- Save money. We'll be able to identify and treat any problems sooner, rather than later, which is usually much less costly than crisis treatment
Rescheduling or Canceling Your Appointment
When you schedule an appointment, we reserve that time just for you and prepare for your arrival. We understand that conflicts can arise unexpectedly, however, we kindly request that you notify us at least 48 hours prior so we have time to contact other patients who are awaiting treatment.
